Fill in your name and date of birth and click Submit.

You will receive a confirmation email to the address you used to sign up.

If you attempt to log into your ºÚ°µ±¬ÁÏÍø MyChart account and can’t remember your password, click the blue “” link under the Sign In button.

Fill in your username and date of birth. Click Next. A one-time code will be emailed or texted to you.

Enter the code onto the form and click Next. You will be prompted to create a new password.

Click Next to complete the process.
