Digital Hospital
A hospital-at-home program that provides care with digital technology and in-person visits. A virtual care team is available 24/7.
Currently only available in the Peoria and Rockford communities.

Vehicles staffed with digital health worker who provide health screenings, connections to resources, technology assistance and enrollment in digital health programs.
King Care-A-Van
A mobile health center connecting individuals to health services and resources in Bureau, Henry, LaSalle and Putnam counties.

We use technology to help manage your health from home. With ºÚ°µ±¬ÁÏÍø OnCall Connect, you can get support between doctor visits. This includes personalized education, a virtual care team available 24/7, regular check-ins and medical equipment, as needed.
Chronic Condition Monitoring
Care for those with the following chronic conditions: asthma, COPD, diabetes, heart failure and high blood pressure.
Pregnancy and Postpartum
Support and education in between regular doctor visits from eight weeks of pregnancy through six weeks postpartum.
